Eligibility Criteria
We strive to serve all individuals 16 years of age or older with a medical or mental condition that requires daily physical assistance, cueing, and/or supervision throughout the entire task of at least one of the following activities:
- Transferring
- Walking
- Dressing
- Eating
- Bathing
- Toileting
Important Note: AFC service must be approved by your primary care physician and insurance carrier.
